LinktCare exists to make the National Disability Insurance Scheme (NDIS) feel less like a system. Participants, families and support coordinators get a clear line to the help they need, and a team that follows through.

Our purpose

A clear line to the help you need.

Thriving Alliance Pty Ltd trading as LinktCare supports NDIS participants from our office based at Level 1/44 Watton St, Werribee VIC 3030, with meetings by appointment. From our Werribee base we support people across Wyndham City Council, City of Melton, City of Brimbank, City of Hobsons Bay, and Geelong inner suburbs.

The support system can feel like a maze of plans, portals and phone tag. We keep it simple. You talk to a person who knows your situation, you get a plan in plain language, and our support workers, support coordinators and recovery coaches do what they said they would, when they said they would.

The values behind LinktCare

Four values shape how we hire, how we plan, and how we show up for every shift.

We deal in specifics. What happens next, who is coming, and when. No jargon and no vague promises.

Held to high standards of quality and safety

As a quality disability support provider, we work within a clear framework of compliance, continuous improvement and participant safeguarding. Here is what that looks like in practice.

NDIS Code of Conduct compliance
NDIS Worker Screening and police checks for every team member.
Ongoing training in participant safeguarding, first aid and person-centred practice.
Feedback, incident reporting and complaints processes that prioritise participants.
Strict privacy and confidentiality protection at every step of support.
What we deliver

Twelve supports across two groups

LinktCare delivers core supports and capacity building supports for plan-managed and self-managed participants. We do not support NDIA-managed plans.

Personal and daily living

Assistance with personal activities and daily life, including high intensity supports and overnight self-care.

Household tasks

House cleaning, laundry, and house or yard maintenance that keeps home running smoothly.

Community and transport

Community participation plus assistance with travel and transport, so plans become real outings.

Respite and short stays

Short term accommodation (STA), including respite, planned properly for everyone involved.

Coordination and recovery

Support coordination and psychosocial recovery coaching, with one clear point of contact.

Skills and employment

Skills development, life transition planning, tenancy support, and help finding and keeping a job.
